We seek an experienced Community Health Worker (CHW) to join our fast-growing team and help grow the mission and impact.
Position Summary
The CHW serves as a liaison between community members and healthcare providers to improve access to healthcare services and promote wellness. This role focuses on outreach, education, and advocacy within the community to address health disparities and empower individuals to make informed decisions about their health. The CHW plays a vital role in delivering culturally competent care and fostering positive health outcomes.
Primary Responsibilities
Community Outreach and Education:
- Conduct outreach activities to engage community members and promote health education and awareness.
- Educate individuals and families on topics such as preventive care, nutrition, chronic disease management, and healthy lifestyles.
- Distribute educational materials and resources to support health promotion efforts.
Health Assessment and Support:
- Conduct assessments to identify community health needs and barriers to healthcare access.
- Provide guidance and support to individuals in navigating healthcare systems, scheduling appointments, and accessing necessary services.
- Advocate for clients and assist them in understanding their rights and options for healthcare services.
Health Promotion and Disease Prevention:
- Develop and facilitate workshops, support groups, and community events focused on health promotion and disease prevention.
- Collaborate with local organizations, healthcare providers, and public health agencies to implement health initiatives and campaigns.
- Encourage participation in screenings, vaccinations, and other preventive health measures.
Case Management and Referrals:
- Maintain accurate records of client interactions, assessments, and referrals to healthcare providers or social services.
- Assist individuals in connecting with resources for housing, food assistance, mental health support, and other social determinants of health.
- Follow up with clients to ensure they receive necessary care and support.
Cultural Competence and Advocacy:
- Demonstrate cultural sensitivity and respect for diverse beliefs, traditions, and practices within the community.
- Advocate for equitable access to healthcare services and resources, addressing social determinants of health that impact health outcomes.
- Collaborate with community leaders and stakeholders to address health disparities and promote health equity.
Required Experience
- High school diploma or equivalent required; bachelor’s degree in public health, social work, community health, or related field preferred.
- Proven experience (typically 1-3 years) in community outreach, health education, or a related role.
- Knowledge of community resources, social services, and healthcare systems.
- Excellent communication and interpersonal skills, with the ability to build rapport and trust with community members.
- Strong organizational and time-management skills, with the ability to prioritize tasks and manage multiple responsibilities.
- Empathy, compassion, and a commitment to serving underserved populations and promoting health equity.
- Proficiency in basic computer skills and ability to use electronic health records (EHR) systems.
- Bilingual or multilingual skills may be preferred, depending on the demographics of the community served.
